The TeleHealth Solution

What makes CareCycle Solutions Different?

Interventional TeleHealth

TeleHealth Coaches do much more than monitor vital signs. We provide targeted intervention including health coaching, medication changes, home visits, physician coordination... and beyond!

Risk Driven versus Diagnosis Driven

Our patients receive TeleHealth based upon their risk of rehospitalization - regardless of diagnosis.

Predictive Model

In partnership with University of North Texas Health Science Center, a predictive model has been developed based on the data of CareCycle Solutions® facilitating target care models based on risk levels.

Critical Care Experienced TeleHealth Coaches

All of our patients are managed by Critical Care Experienced TeleHealth Coaches. We require this level of expertise to assure that our clinicians are confident in their assessment skills and assertive in their decisions. Our TeleHealth Coaches all work in our central Vital Station and monitor patients 365 days per year. The centralized model allows for greater quality control and promotes an environment of dynamic patient conferencing. Each patient benefits from the experience of the whole.

Focus on Medications

This chart shows that 40% of the reasons for rehospitalization are related to medication management and compliance. We focus on medications with every patient contact - what, how much, why, how and when.

Causes of Hospital Readmission for HF

Data • Data • Data

CareCycle Solutions® has supporting data for over 14,000 patients and 1.85 million patient days. Clinical outcomes can be reported by hospital, diagnosis, payer, demographics and more.

Relationships that Build Personalized Care

We intentionally build personal relationships between patients and TeleHealth Coach Teams by ensuring that the same TeleHealth Coaches are consistently monitoring the same patients. It is this knowledge of the stable state of a patient’s health which can enable a TeleHealth Coach to recognize instantly when something is out of the ordinary.
